o College Transcripts (Official copy mailed to us will be required if selected to interview)

o Three Letters of Recommendation (one from current or former employer)

o Air Force Officers Pilot Candidate Scores (AFOQT)

o Pilot Candidate Selection Method Scores (PCSM)

o Private Pilot’s License (required)

MILITARY CANDIDATES (to include prior military where applicable):

-DAF1288 or DD0368

o One letter of recommendation must be from current commander

o Copy of all EPRs/OPRs (including prior military)

o Copy of DD214s (including prior military)

o All Form 8s (if applicable, including prior military)

o AF Form 942 (if applicable, including prior military)

o Physical Fitness Assessment (Fit to Fight)
